Attached is a PO Form that can be downloaded to your computer, filled out and emailed directly to the ticketing system.
- Click on the PO Form attachment
- If the form opens in your browser, download the form to your computer
- Complete the information on the form. To expedite your request please consider all fields to be required.
- Submit a New Ticket via the Portal OR E-mail the completed form to challengerlifts@soe.freshdesk.com.
- Include the word Part, Service or Warranty in the Subject Line for Parts orders.
- Do not include these key words for all other Lift or Accessory orders.
Don't forget to DOWNLOAD the file to your computer, editing in your browser does not guarantee the content on the form will be saved.
